DYOR Restaurant-Bar Company Info and Reservation Brochure

This is my second time to design a brochure but my first for a restaurant-bar. The theme of the brochure is black and white and it must have the company profile and other information.  Also, the reservation form must be part of the brochure.  The brochure will be folded twice with the company logo as the front side and the weekly schedule as the back side of the brochure.
